Your role as a Nurse Practitioner
The mission of the nurse practitioner is to ensure the evaluation, care, and referral of patients consulting virtually by collaborating with a multidisciplinary team. You will closely collaborate with our virtual team to ensure a seamless experience all throughout the patient care journey.
What you'll be doing
- Conducting remote assessments via the Dialogue Care Platform where you will evaluate symptoms, diagnose and provide treatment for both common and complex conditions
- Delivering high-quality patient care by offering timely medical advice and identifying urgent symptoms
- Creating personalized care plans tailored to your patient's needs
- Collaborating closely with an interdisciplinary team of doctors, nurses, care coordinators and care managers, psychologists, etc to provide comprehensive, high-quality care
- Educating and supporting patients on preventative care, health management and lifestyle changes
- Referring patients, when needed, to specialists or allied healthcare professionals and community services
- Maintaining detailed records of patient interactions, treatment plans, and follow-ups that adhere to regulatory standards
- Contributing to internal projects or initiatives

- Must be comfortable consulting in French or English
- A minimum of 8 hours of availability per week is required.
- Evening and weekend availability is required across all provinces. Candidates who prefer to work exclusively evening and weekend shifts are also welcome to apply.
- This role is open to Full-Time employees (32 hours or more), Part-Time employees (21 hours), and Part-Time contractors.
- Master's degree in Nursing, advanced practice option and a complementary diploma in primary care
- Hold a valid and active medical license in either of these provinces; Ontario, Manitoba, Alberta, Prince Edward Island, New Brunswick, British Columbia, Newfoundland and Labrador, Nova Scotia or Saskatchewan.
- Bilingualism (French and English) is considered an asset for practitioners licensed in New Brunswick and Ontario
